FUSE corp Mainframe: Blank

This is for the use of only.

So yeah here's a blank one for you all to fill out if you want. Just read the instructions and you'll be fine.

Welcome Fuse Corp employee to the Employee Files Mainframe or EFM. This is a tutorial program on how to fill out your file for the mainframe to keep track of. This system is encrypted and protected from all intrusions including internal ones so your file is safe. Read and follow the instructions.

-First put in a picture of yourself. This can be the same picture you have for your IDs or a new one. Next put in the obvious information under the picture and you ID number up top. Please use the same ID number on your ID card as to avoid confusion of the system.

-Next under Latest Experiments put the last 10 experiments you’ve created with the latest one on top in the spaces provided. Beside that you will notice the danger lvl column. In this column you will give what level of danger that experiment possesses in relation to other employees along with a color to match in the spaces provided. Starting at the lowest would be:
Minimum=Blue
Low=Blue-Green
Medium=Orange
Med-High=Red-Orange
High=Red
Extreme= Pink
We trust that you will be honest in evaluating the danger level of your experiments.

-Next you will list the last 6 items you have created with that latest one on top.

-Next under the Summary section, in the spaces provided, please fill out the total number of experiments you’ve done. This includes those created through collaborations with other scientists. Please then put your current average success rate in creating both experiments items. This will not affect your standing in the company. Then please state how many assistants work with you in your lab. This includes both FUSEmon assistants and human assistants.

-In the History section you may give a brief background of yourself provided it doesn’t exceed the space provided.

-Lastly in the Project History section please list all projects you are currently working on, have worked on, and is awaiting approval. Please be sure to include the name of the project, its purpose, and its current status of Complete, Ongoing, Awaiting Approval, Suspended, ect.
